THERE WILL BE SPOILERS!!!!!!
OK, this is for real your last chance to turn back! Don't spoil DEATH ROW for yourself before reading it!
!!!!!!!!!SPOILERS!!!!!!!!!!!!!
My own interpretation of the ending:
!!!!!!!SPOILERS!!!!!!
(Last chance to keep from ruining it for yourself!)
All of the "Before" chapters are real memories of Talia's life. Talia tried to kill her husband, but she was able to warn him in time, and he survived. Unfortunately, she was so distracted that she was in a car accident on the way home, and had a severe traumatic brain injury, resulting in a coma.
The scenes on death row are a "coma dream." She dreams about being on death row because she feels so guilty about what she tried to do to her husband. People in comas often report that they are aware of things happening around them, so Talia incorporated things in the hospital into her dream about death row. The nurse Rhea becomes a guard, the doctor becomes her lawyer. The chaplain giving her last rights at the hospital is the same one giving them to her on death row. The beeping sound she hears on death row is the sound of her life support monitors in the real world. The pain in her throat is due to the endotracheal tube down her throat for the ventilator.
In the final chapter, it is revealed that Talia is in a coma, and they are taking her off life support. They tell Noel that once they take her off life support, she will be free.
In the epilogue, Talia is dreaming again. She dreams about an alternate world in which she never got into the accident, and everything worked out for her. She still feels the pain in her throat and hears the beeping of the life support. When the beeping stops, that means they have taken her off life support. The pain also goes away and she is "free" as promised, which allows her to finally kiss Noel.
I have heard a lot of different interpretations of the ending, and I'm OK with that! In my own head, I can imagine two interpretations of the epilogue: Talia had entered the afterlife when life support was removed, or else she was in some brief transitional state between life and death. I've also heard interpretations where people thought Noel was dead as well, although that was not my personal intention.
I hope this helps!!!
